When you visit Cape Regional Eye Center, PLLC, you'll receive comprehensive eye care. The information below will help streamline your first visit.
On Your First Visit
Making an Appointment
To make an appointment please call our office at (731) 286-2801 or toll-free (866) 591-4362 (IDOC). Our receptionist will be happy to arrange a suitable appointment time for you. Patients are seen as close to appointment time as possible.
Walk-in emergencies will take priority.
Forms and Information
When you come for your scheduled appointment, you will be asked to fill out approximately 6 pages of pertinent information. These forms may be downloaded and filled out at home prior to your visit.

You will be asked to bring all your insurance cards, your driver’s license, a list of medications you are currently taking, and your glasses. If we can get your insurance information prior to your visit, then verification of benefits can be checked ahead of time to expedite your check-in process.
Your Appointment
It is always a good idea to bring ONE person with you since we might dilate your eyes and you would be unable to drive yourself. Please allow approximately two hours for your routine eye exam experience. There may be additional office time needed to have any special testing or procedures.
After registering upon arrival, a technician will call you to the preliminary testing area. This is where your initial exam will start. Your medical history will be obtained and preliminary testing will be performed to prepare you for your examination by the doctor.
If you are unable to make your scheduled appointment time, we ask that you notify us at least 24 hours in advance. This will allow us to give your appointment time to someone else.
Treatment Clearance Information
If you are being treated for certain conditions, we'll need clearance from the doctor doing that treatment before we can treat you. If any of the following pertain to you, please let us know.
- Major surgery < 1 month
- Recently diagnosed diabetes < 3 months
- Heart attack < 6 months
- Stroke < 6 months
- Heart surgery < 1 year
- Transplant of any kind < 1 year
- Seizures < 1 year
- Lung cancer
- Dialysis
- Hepatitis
Insurance Billing
Cape Regional Eye Center, PLLC accepts Medicare, Blue Cross, and most other insurances. You are expected to bring all insurance cards and a picture ID, such as a driver’s license. We require payment of co-pays, deductibles, and any outstanding balances at the time of the visit prior to being seen by the doctor.
We will be happy to file your insurance for you. If you are not sure if we accept your insurance, please feel free to call the office at 731-286-2801 or toll-free 866-591-4362 (IDOC) and someone will be happy to assist you. We DO NOT ACCEPT vision plans, which are plans that pay for the exam and glasses.
